Barbara Schubert will register your child with The Staten Island Developmental Disabilities Office (DDSO) at 718-982-1904
Richard Monck of the DDSO can be reached at 718-983-5232 let him know that you would like to begin the process of obtaining Medicaid Waiver Services for your child; he will let you know what needs to be done.
Ellen Blackman of the DDSO can discuss Medicaid Service Coordination. Her number is 718-982-1905.
Mary Beth Fadelici of Parent to Parent NYS is a Medicaid Service Coordinator Education and Information Parent Educator and can be reached at 1-800-405-8818.
Should your child meet the eligibility criteria for services provided by The New York State Office of Mental Retardation Developmental Disability (OMRDO) they would be able to receive a variety of services including Medicaid Waiver Services. Medicaid Service Coordination, Family Reimbursement and Respite services.
For Family Reimbrusement you can contact Leroy Smith 718-982-1903. He will send you an applicaiton.
For Respite/Homecare service information contact Barbara Schubert or Ellen Blackman.
